Community Services

There are a number of community and private services that may need to be arranged prior to you leaving hospital. Access to the Case Manager - Community can be made through nursing staff to ensure your needs after discharge are addressed and ensure effective transition from hospital to "home". A range of services/procedures are available and coordinated in consultation with case managers and service providers.
