Community Services
There are a number of community and private services that may need to be arranged prior to you leaving hospital.
Access to the Case Manager - Community can be made through nursing staff to ensure your needs after discharge are addressed and ensure effective transition from hospital to "home". A range of services/procedures are available and coordinated in
consultation with case managers and service providers.
Services that can be accessed include:
- Aged Care Assessment Team (ACAT)
- Breast care nurse
- Cardiac Rehabilitation
- Colostomy Association
- Community or private nursing
- Dietician
- Home and Community Care (HACC)
- Home Help
- Lactation Consultant
- Mastectomy Society
- Meals on Wheels
- Outpatient Appointments
- Specific Associations/support groups e.g. Alzheimer's Association
- Sudden and Neonatal Death Syndrome (SANDS)
- Women's Health Counselor
Discharge Procedures
The date of discharge will be arranged in consultation with your doctor, nursing staff and yourself. Staff will help you make necessary personal arrangements. We recommend that someone accompany you home after discharge, if possible.
Before discharge please remember to:
- Collect the medication you brought to the hospital, as well as any new treatment commenced after admission.
- Take home all x-rays, scans, ultrasound films
- Ensure all valuables are collected and taken with you
- Collect details of doctor's appointment and/or letter.
- Collect letter for Community Nurse if applicable.
If you have any transport problems, please advise the nursing staff caring for you. In some circumstances discharge may require the services of the Ambulance or similar transport. Ward staff will be able to advise you and organise this for you. (See Ambulance Section)
On departure, a member of staff will escort you to Patient
Administration Services, Front Foyer, Main Entrance 1.
The staff will then finalise your account so that financial clearance from the Patient Administration Services Department can be made before departure. Departure time is before 10.00 am. This assists us with the accommodation of incoming patients. If you are unable to be picked up by 10.00 am, please advise the nursing staff of any discharge difficulties.
Departure time for patients being transferred via road or aerial ambulance is at the discretion of the service.
